Patient Portal
My Chart Puts Your in Control of Your Health
All Trinity Medical patients are encouraged to activate MyChart through our shared Catholic Health system. This easy-to-use, secure patient portal allows you to make and manage appointments and medications, communicate with your doctors, access test results, and more. To get started with MyChart, follow the steps below.
- Visit mychart.chsbuffalo.org or use the button below to visit the login page. Once you’re on this page, you’ll be prompted to login or sign up. If this is your first time using a Trinity or Catholic Health patient portal, click the “Sign Up” button below the login prompt.
- Enter your activation code. Your provider’s office may have provided you an activation code. If you have this code, you’ll enter it here, followed by the last 4 digits of your social security number and your date of birth.
- OR sign up with additional information. You may not have been provided and activation code, or you may have misplaced it. If so, click “sign up with information” instead, and fill out that form. The required information MUST match the information you provided your doctor’s office.
- Explore the app. You can also download the mobile app in the Google Play or Apple App stores to keep the patient portal at your fingertips.
