For patients who were recently hospitalized or diagnosed with heart failure, Buffalo Heart Group has a dedicated team of physicians, mid-level providers and registered nurses to monitor and follow your condition. The doctors and staff of the Congestive Heart Failure (CHF) Clinic follow guidelines set by the American College of Cardiology and the American Heart Association to try and improve symptoms and minimize readmission to the hospital for our patients.
Heart Failure Management is a team effort – making you and your physician key players. Our providers and staff are here to provide specialized care and education to help you better understand your condition and manage your health. Our CHF clinic will provide ongoing follow up as long as it is needed – the goal is to help you live well with CHF and continue doing the things you enjoy.
